FINAL REPORTS for Organizations: Please Read

There are two ways to submit the Final Report. To determine which to fill out click here.

If you are required to submit the Final Report Online, use the egrant application, click on the link (on this page) that says Online Application.

FY08 Continuation Groups Only

For Groups that are part of the Continuation Program you are required to submit the Paper Format. Click here for the FY08 Continuation Final Report Form MS Word Document.

Penality for not filing the final report on time

Grantees who fail to file the required report within 90 days of the deadline will forfeit final payment and jeopardize future awards. Payments on future pending grants will be penalized by forfeiture of each quarterly payment for each quarter delay in filing the Final Report. Failure to file Final Report on previously due grants prior to December 31, 2008 will prevent organization for applying for funding in next fiscal year.

NEW FOR CONTINUATION GRANTEES: unless you hear otherwise from our office, you are not required to submit support materials (see below items) for your Final Report at this time. Although you are not required to submit support materials, you are still required to produce these materials if an audit is requested by the City or Houston Arts Alliance. As stated in your agreement, these records need to be available for three years. You will still have to list how Houston Arts Alliance funds were spent for the grant period.
